Identification cards or insurance information are not required, however all registrants will need to provide contact information and complete a consent form prior to vaccination. Please note that your second dose will be automatically scheduled for the same time 21 days later (May 15 or 16, 2021). If you cannot commit to the second appointment, please consider another vaccination option.

Encourage action for those that waited their turn. Vaccines are widely available for Iowans age 16+. Share scheduling links or suggest searching for open appointments using Vaccine Finder or Vaccine Hunter. Many locations now welcome walk-ins.
